WISEWOMAN Program
WISEWOMAN (Well-Integrated Screening and Evaluation for WOMen Across the Nation) helps participants understand their cardiovascular disease (CVD) risk factors and make healthy lifestyle choices.
WISEWOMAN is a federal program funded by the Centers for Disease Control and Prevention (CDC) and administered in Michigan by the Michigan Department of Health and Human Services.
The information on this page explains various aspects of our program. If you are not able to find what you are looking for, please email us at MDHHS-MIWISEWOMAN@michigan.gov. Call 844-446-8727 (TTY 711) if you have questions or need help enrolling. Interpreters available.

What Is WISEWOMAN? Who Can Participate?
The Michigan WISEWOMAN program has three main focus areas with relation to its participants.
- Identify and communicate risk factors for cardiovascular disease (CVD), stroke, diabetes, and other chronic diseases. Enrolled participants undergo a health assessment and risk reduction counseling. These services give participants the opportunity to decide if they would like to set a goal to improve their diet, physical activity, and other lifestyle behaviors.
- Encourage healthy lifestyle choices. Health coaches work with each participant to make lifestyle behavior changes that will help to prevent, delay, and control cardiovascular and other chronic diseases.
- Address health equity and social justice in the community.
- Local WISEWOMAN agencies address health equity by identifying priority populations in their service areas and getting them into WISEWOMAN. Priority populations may include:
- Women with disabilities;
- Women who do not speak English;
- Lesbian, gay, bisexual, and transgender (LGBT) women;
- Racial or ethnic minority women; and
- Women whose citizenship or immigration status is not settled.
- Once in the program, many participants find it difficult to think about making healthy lifestyle choices when they are having trouble paying rent, utilities, or buying food for their families. These are social justice issues. Michigan WISEWOMAN makes referrals to Michigan 211, where participants are connected to a community navigator who helps them access social services. Michigan WISEWOMAN also helps participants stay engaged in the program by offering barrier reduction tools.
Who is eligible for the WISEWOMAN Program?
Those who are eligible for the WISEWOMAN program include women, transgender, or gender nonconforming individuals (taken from CDC guidance) who are:
- uninsured or underinsured;
- at or below 250% of the Federal Poverty Guidelines;
and
- 35 to 64 years of age.
Eligible clients who will benefit from this program include those who are at increased risk of heart disease and need preventive health screenings as part of their wellness routine.

Women's Health Partnership
Heart disease and cancer are the leading causes of death for Michigan women. To address this issue, the MDHHS Cancer Prevention and Control Section offers screening programs especially for women:
- The WISEWOMAN Program provides screening for heart disease risk factors and health coaching to make lifestyle changes.
- The Breast and Cervical Cancer Control Navigation Program (BC3NP) provides breast and cervical cancer screening to eligible women.
- The Program for Breast Cancer in Young Adults provides educational and quality of life resources for cancer survivors, metastatic thrivers, caregivers, and health professionals.
The Cancer Section began the Women’s Health Partnership to partner with organizations to help promote these screening programs. Partner organizations:
- enroll women for lifesaving health screenings;
- engage women in services for themselves and their families; and
- help women take advantage of free health services in the communities they serve.
