What is a Plan of Safe Care?
A Plan of Safe Care (POSC) is a personalized tool to support pregnant and parenting people impacted by substance use, their infants, and any other household members. A POSC focuses on family strengths, healthy pregnancies, and keeping families safely together.
The purpose of this website is to support families and professionals co-creating a POSC. It is intended to provide templates, tools, and key resources that may be helpful in navigating the journey of parenthood while experiencing the impacts of substance use.
What are the desired outcomes of a Plan of Safe Care?
POSC desired outcomes include:
- Ensure the safety of infants and children.
- Identifying prenatal substance exposure and engaging families in care.
- Promoting substance use disorder treatment completion and engagement in recovery.
- Ensuring children live in safe, stable, and nurturing environments.
- Keeping families together whenever possible.
- Safely reunifying families as quickly as possible after removal has occurred.
- Connecting families to desired services.
- Securing involvement of family members and support persons.
