MDHHS issues new Michigan Physician Orders for Scope of Treatment form, trainings

Starting Friday, June 30, Michiganders who use the Michigan Physician Orders for Scope of Treatment (MI-POST) form to help guide their care during a medical crisis will need to complete an updated form due to changes in administrative rules.

The Michigan Department of Health and Human Services (MDHHS) has issued a new MI-POST form, which can be downloaded from the MDHHS website. The update was made due to administrative changes to the document for clearer understanding. MI-POST is an optional medical order that is part of the advance care planning process and includes choices about cardiopulmonary resuscitation, critical care and other desired medical interventions. It is intended for individuals with serious advanced illness or frailty.

“The MI-POST form allows families to guide care for individuals who are not able to direct their own care during a medical crisis,” said Dr. Natasha Bagdasarian, MDHHS chief medical executive. “It’s a critical component of advance care planning.”

Individuals who have used the MI-POST form in Michigan are required to use the new MI-POST form version created by Public Act 154 of 2017. MI-POST forms completed on the old version of the form before Friday, June 30, will be considered valid one year from the completion date or until a circumstance that requires reaffirmation or revocation of the MI-POST form, as detailed on the form instructions.

Once it is required that the MI-POST form be reaffirmed, an updated form should be completed. The MI-POST form is available in Spanish, Arabic and English.

To help licensed care facilities, clinicians and hospital care teams and patients, families and caregivers understand the MI-POST form and its usage in the advance care planning process, the MDHHS MI-POST Committee has developed trainings that are available online.
