The Michigan Department of Health and Human Services submitted its Healthy Michigan Plan §1115 Demonstration Waiver Extension Request Amendment to the Centers for Medicare & Medicaid Services (CMS) for review on
September 10, 2018. MDHHS has amended its Demonstration Extension Application previously submitted to CMS
on December 6, 2017. The Demonstration Extension Application Amendment is subject to approval by CMS and can
be accessed below.
