Hello Colleagues!
Please find attached a guide for information to include when requesting addition of providers to cover therapies. The Customer Support Section has been receiving requests to add therapy providers, but many requests are missing key information to determine if the request can be honored. Providing the information listed in the attachment will allow a quicker response to requests, especially if the request requires review by the medical consultants.
Questions should be directed to your assigned analyst, or you may contact the appropriate Customer Support Managers for your assigned analyst.
