Reporting Changes to AGAO

Address Changes
Address changes must be submitted in writing by email, mail or fax. Emailed address change requests can be sent to MDHHS-CSA-AGAO-Ongoing-General@michigan.gov. The written request must include the following:
- Parent’s/guardian’s name(s)
- Phone number
- Child’s name(s)
- Child’s date(s) of birth
- Child’s SSN(s) (needed if you are leaving or reentering the State of Michigan)
- Complete Old Address
- Complete New Address
- Parent’s/guardian’s Signature(s)
Your SIGMA account must also be updated to your new address. AGAO cannot make changes in SIGMA. Parents/guardians must update their account online by going to https://sigma.michigan.gov/PRDVSS1X1/Advantage4. Your address MUST be updated in SIGMA before AGAO can process your address change request.

Name Changes
To update your name with AGAO, please submit a copy of a legal document (Example: marriage license, divorce decree, driver’s license/state ID) with your new name. This can be mailed, faxed, or emailed to MDHHS-CSA-AGAO-Ongoing-General@michigan.gov.
Your SIGMA account must also be updated with your new name. AGAO cannot make changes in SIGMA. Parents/guardians must update their account online by going to https://sigma.michigan.gov/PRDVSS1X1/Advantage4. Your name MUST be updated in SIGMA before AGAO can process your name change request.

Payee Changes to Secondary Parent/Guardian
Please submit the following documents by mail, fax, or email to MDHHS-CSA-AGAO-Ongoing-General@michigan.gov:
- Written request to be made payee with BOTH parent signatures
- If parent/guardian states they cannot get the other parent/guardian signature on the written request, a court order showing the parent requesting the payments has full custody of the child may be used.
- If payee change is due to the death of the current payee, please provide a copy of the death certificate.
- Driver’s license/state ID
- Copy of SSN
New parent/guardian must register in SIGMA (State of Michigan’s payment system) and provide their SIGMA vendor code to AGAO. To register in SIGMA, go to: https://sigma.michigan.gov/PRDVSS1X1/Advantage4.

Adoptive/Guardianship Child Out of Home
Please notify AGAO as soon as possible when an adoptive/guardianship child is no longer living in your home. Please email MDHHS-CSA-AGAO-Ongoing-General@michigan.gov.
Examples:
- Residing with a family friend, relative, or guardian
- Residing in detention
- Residing in inpatient and/or residential treatment facility
- Placement in foster care
- Child reported as runaway
