Annual Reports

The annual report is a document which adoptive parents and guardians are required to submit once a year to AGAO for the purposes of verifying continued eligibility for adoption and guardianship assistance and for funding purposes.

Approximately one month before the child’s birthday, the annual report will be mailed to the address on file with our office. This means that if you have more than one adoptive child or child in a guardianship, you will not receive their annual reports all at once, but rather at the time of each individual birthday.

If your child’s birthday has passed and you have not received an annual report, please email MDHHS-CSA-AGAO-Ongoing-General@michigan.gov to ensure AGAO has your current address on file.

To ensure your annual report is received in a timely manner by AGAO, it is recommended you complete your annual report online using the Provider ID and PIN number listed on the annual report letter. Please note that your Provider ID always stays the same, but each child receives a new PIN number each year.

  • Please mark yes to this question if the child is currently residing with the adoptive parent(s) or guardian(s) who have a signed agreement with AGAO. If the child is residing with anyone who does not have a signed agreement with AGAO, please check no.
