Construction prequalification
Contractors must be prequalified to bid on or perform MDOT projects, including work such as bridge, road, utility, electrical, demolition, painting, signing, guardrail, landscaping, pavement marking, and rail construction. Prequalification process
The video guides construction contractors through the process of electronically submitting prequalification information.
- Obtain a prequalification application packet.
- Download the required forms; or
- Request an application packet online
- Complete all forms and attachments in the prequalification checklist packet.
- Ensure all required original signatures and notary information are complete.
- New contractors must include the following with the application:
- Articles of Incorporation or Articles of Organization
- W-9 Form: Request for Taxpayer Identification Number and Certification
- Attach optional equipment appraisal

For new contractors and renewals not meeting 1313EZ requirements. - Construction prequalification renewal application (Form 1313EZ)
- Construction prequalification reference (Form 5107)
For new contractors or if requesting additional work classifications. - Account receivable verification (Form 1309)
