Qualified Forest Program Application | This form is to enroll land in the Qualified Forest Program (QFP). Use the QFP Transfer Form if the land you have purchased is enrolled in QFP. Applications can be submitted year-round. Submitting earlier in the year gives you time to correct any issues. Incomplete applications received by September 1 do NOT qualify for the exemption in the next tax year. Incomplete applications with six months of inactivity will be cancelled and a $50 fee will be charged to re-activate the application. Township assessor must approve land splits and combinations in writing before application can be processed. Owners must include last 4 digits of their SSN so QFP staff can confirm their identity to provide information. |
Stand Summary and Harvest Schedule | This form summarizes the forest cover types and commercial harvests that are prescribed by your Forest Management Plan (FMP). The total acres in this form must be equal to the number of acres being applied for QFP enrollment. Only list commercial harvests in the schedule below. Stand type, tree size, and harvest type fields must use the attached QFP Inventory cover type, tree size, and harvest types. Only one stand type, tree size, and harvest type may be used per field. |

Treasury Form 5020 | Assessor’s confirmation of agricultural use property acreage for the purpose of determining if land may be exempt as Qualified Forest Property. Part 1 of this form is completed by the landowner. Part 2 is completed by the Assessor. The landowner will use this completed form as part of their application to the Qualified Forest Program. |
