Report a Vehicle Accident Involving a State Vehicle

In the event of a collision or incident of vandalism, the driver should make notes about the accident. An envelope with vehicle information, typically kept in the glovebox, contains information about what should be recorded. This information will be needed when filing an accident report, and departments may require that a driver submit such a report.

The driver must:

  • File a report with local police.
If the police cannot make an on-scene investigation, the driver must go to the police department and file a report. If the police will not take a report, the driver should simply write a statement which explains that circumstance.
  • Mail or fax a police report or your personal statement.
Send this report or statement to:

Wheels Inc.
State of Michigan Repair Services (MAP) Collision Management
666 Garland Place
Des Plaines, IL 60016

or send report via fax to:
Fax: 847-699-8491

  • Request reimbursement for any cost incurred for obtaining copies of police reports.
Request this from your department through the designated travel reimbursement system.
  • Report the accident to the MAP Fleet Services Collision Center.

MAP Fleet Services Collision Center
Phone: 1-800-937-8149.

The Collision Center will:

  • Complete crash report over phone.
  • Assess damages and determine repair action.
  • Arrange for towing when necessary.
  • Identify vendor for repairs.
  • Authorize all repairs and assure timely completion.
  • Arrange for alternate transportation if necessary.
  • If provided a rental vehicle, contact your Department Vehicle Coordinator the next working day.
The Department Vehicle Coordinator will determine the most cost-effective means of travel during assigned vehicle downtime.

Report a Vehicle Accident Involving a Rental Vehicle

Under Act 204, P.A. 2012, the State of Michigan as a self-insurer cannot insure rental vehicles.

An employee must purchase rental insurance from the rental agency. The claim must be filed with the rental company per the rental agreement.

Report a Vehicle Accident Involving a Privately-Owned Vehicle

Under Act 204, P.A. 2012, the State of Michigan as a self-insurer cannot insure employee personal vehicles.

An employee must submit a claim with their personal auto insurer.

Enterprise Risk Management encourages employees to rent a vehicle from the State’s Motor Pool Reservation program to avoid incurring a personal auto insurance claim.
