Name Change Information

  • Please email the Licensing Division at CSCLOnline@michigan.gov and request a "Legal Name Change". Include the following in your email:

    • Full name of licensee as printed on the license. 
    • New full legal name. 
    • License number. 
    • A copy of your legal documents showing the name change.

    The Department will notify you if additional information/documentation is needed.

    • Full name of your sole proprietorship as printed on the license. 
    • New full legal name. 
    • License number. 
    • A copy of your legal documents showing the name change.

    The Department will notify you if additional information/documentation is needed.
