You must complete and submit an AFC Camp renewal application annually prior to the license expiration date to the Department. All license fees are nonrefundable. Make check or money order payable to the STATE OF MICHIGAN.
AFC Camp Renewal Application (BCHS-3328 Adult Camp P)
The renewal fee for an AFC Camp is $200.
Change of Leadership
If your camp has a change of licensee, director, program director, or camp administrator, contact your Licensing Consultant to request a “licensing Record Clearance” form (BCAL 1326) with the RI-030 fingerprint form.
