AFC Camp Licensee

AFC Camps are issued a license for a 12-month period. Statutory requirements for continued licensure require an onsite inspection every year to determine ongoing compliance with statute and administration rules.

Therefore, licensees must submit a renewal application to the State of Michigan annually.

BACKGROUND CHECK INFORMATION

The license, licensee designee, director, program director, and administrator must have a fingerprint background clearance check.  You must contact your Licensing Consultant to request a licensing record clearance form (BCAL-1326-Adult Camp) and a RI-030 fingerprint form. You must complete these two forms and schedule an appointment at an Identigo location to be fingerprinted.

Camper Health and Fire Incident Reporting

A written report must be submitted if a camper has an accident or illness that results in being sent home, an overnight stay in a hospital, clinic, or results in a death of a camper.  In addition, fire occurrences that result in injury, loss of life, or facility damage more than $1000 shall be reported.

In either case, the camp shall complete the department’s incident report and notify the department within 48 hours of the occurrence.
