State Licensing

This page is designed to assist providers only through the state licensure process, as well as provide additional resources to maintain compliance. For federal certification requirements, click here.

State licensing is required for nursing homes, county medical care facilities, and hospital long term care units.

A state licensure application (BCHS-HFD-100) is required to (Licensure fee required):

• Begin operation of a new facility ($2,000 application fee/invoice required) - Include Appendix C & Appendix D
• Change of ownership (CHOW) of an existing facility - Include Appendix C
• Increase in bed capacity 
   

A state licensure application is also required to (No fee required):

• Change name of facility
• Change location of facility - Include Appendix C
• Change in bed designations
• Decrease in bed capacity

• Fee: $500, plus $3 per bed after the first 100 beds
• Annual renewal invoices sent in June of each year
• License renewals must be paid online no later than July 31^st

Other state agencies may require approval prior to licensing actions. Contact the following state agencies to determine whether their approval is required.
