To earn credit for time spent teaching, please complete the attached form and return to the MiFellowship team of the Student Loan Repayment Division (SLRD).

Individuals who received assistance from the Mi Future Educator Teacher Fellowship (Fellowship) program signed the Commitment to Teach Agreement (CTMA). In this agreement, individuals promise to provide the Student Loan Programs Fellowship team with current contact information.
You must notify the State of Michigan Department of Treasury if any of the following events occur at any time after a MI Future Educator Fellowship you received is converted to a Loan:
- You change your address or telephone number;
- You change your name (for example, maiden name to married name);
- You have any other change in status that would affect your loan
If you are still enrolled in school, you can log into your MiSSG Student Portal to change your name, address, phone number, and/or e-mail.
If you are no longer enrolled, you can contact the SLP team by calling 888-643-7521 (when calling, mention you have a “Fellowship”) or by e-mailing MiFellowship@Michigan.gov to update all information.
Prior to beginning any new teaching assignment, you must provide the State of Michigan Department of Treasury with a self-certified Employment Verification Form. This form will be provided to you via U.S. mail annually. You can also obtain this self-certification form online. With this information, SLP will verify your employment status with your school or educational service agency directly and annually. You must self-certify before starting your teaching service or any time you change schools or educational service agencies within a reasonable amount of time.
