10.07.03: Correcting the supplemental TDP agreement

In the event a Supplemental TDP Agreement (R0654C) form is submitted with an incorrect date on line 6, you are allowed to make a correction to the form.

  1. Correct the date on line 6.
  2. Initial and date your correction.
  3. Write "AMENDED" boldly across the top of the form. 
  4. Fax a paper copy of the agreement to ORS at 517-284-4416. You do not need to submit the supplemental agreement to ORS on the TDP agreement file.

Note: An amended supplemental agreement will not be processed if it attempts to lower the scheduled deduction amount below the amount already on file.
