Language Services Complaints
If you had trouble accessing state services because the help you needed in your language was not provided, you can file a complaint related to language access noncompliance. This includes if a state department or agency did not offer an interpreter or translated documents.
Steps to File a Complaint

1. Complete the Complaint Form
- Online Form: You can fill out and submit your complaint directly through our online Language Access Non-Compliance Complaint Form.
- The online form is available in 10 languages: Arabic (العربية), Chinese - Simplified (中文(简体)), English, French (Français), German (Deutsch), Japanese (日本語), Korean (한국어), Russian (Pусский), Spanish (Español), and Ukrainian (Українська). To choose your preferred language, use the drop-down menu in the upper right corner of the form page.
- Downloadable Form: If you prefer, you can download and complete the Language Access Non-Compliance Complaint Form, which is available in 14 languages: Arabic (العربية), Bengali (বাংলা), Chinese - Simplified (中文(简体)), Dari (فارسی/دری), English, French (Français), German (Deutsch), Japanese (日本語), Kinyarwanda (Ikinyarwanda), Korean (한국어), Pashto (پښتو), Russian (Pусский), Spanish (Español), and Ukrainian (Українська). Once completed, you can:
- Email it to SOM-Language-Access@michigan.gov or
- Mail it to Elliott-Larsen Building, 320 S. Walnut St., 6th Floor, Lansing, MI 48933

2. Submit Your Complaint
- Online: Submit the completed form directly through the online Language Access Non-Compliance Complaint Form.
- By Email: Send the completed downloadable form to SOM-Language-Access@michigan.gov
- By Mail: Mail the completed form to Elliott-Larsen Building, 320 S. Walnut St., 6th Floor, Lansing, MI 48933.
- By Phone: If you need help completing the form or prefer to file your complaint over the phone, call 517-335-1181.
When to File a Complaint
- Timeframe: Complaints should be submitted within 180 days from the date of the incident to ensure prompt investigation and resolution.
- Ongoing Issues: If the issue is ongoing, file your complaint as soon as possible.
