Car Seat Distribution Program
As part of a temporary initiative, Medicaid Health Plans (MHP) are working to facilitate the distribution of car seats to TANF-eligible, MIHP families. This process will be coordinated between the MIHP Provider and the MHP, but some key elements are included below. This program ends September 30, 2025 or once the allocated funding has been exhausted.
Eligibility
- MIHP-enrolled, within a Medicaid Health Plan (not fee for service), and
- Eligible for Michigan Temporary Assistance to Needy Families (TANF)
Process and Form
To request a car seat for an eligible beneficiary, MIHP providers will complete the Infant Car Seat Distribution Program Form and submit it directly to the applicable Medicaid Health Plan. Further details, including car seat delivery, are to be coordinated between the MIHP Provider and the Health Plan. The MHP is responsible for ordering the car seat, coordinating delivery, and establishing a communication process with MIHP Providers.
- Form: Car Seat Distribution Program Self-Declaration of Low-Income and/or Public Benefits form
- If after completing the form, the beneficiary is found to be eligible and in need of a car seat(s), submit the form to the beneficiary’s Health Plan. See Car Seat Contact List for submission information below.
- When the car seat is delivered, update the form and submit it to MDHHS MIHP at the specific email or fax below. Please submit forms monthly.
