Roles for External Users: What "user role" do I request as an external user?
External users registering with the Single Sign-on system for using DCH CON E-Serve online application and information system must select either "Applicant" or "Guest" role only.
E-Mail Addresses:
Can I enter multiple addresses for the agent E-mail address field?
Yes, you can enter multiple e-mail addresses separated by comma (,).
Viewing LOIs Submitted by Others:
Can I view LOIs submitted by other applicants?
Yes. Click on the link ‘CON History' under Browse to view any Letter of Intent submitted by other applicants. Please note that filing LOIs online has been available only since 2006.
Submission of LOI: How do I know if the LOI is submitted to the Department?
When you complete the last screen of the LOI, click on the button ‘Save/Submit LOI'. You will see a confirmation message on screen, which means your LOI has been successfully submitted to the Department.
LOI Processing: How do I know if the LOI is processed by the Department or access the application components selected for my project?
The agent listed on the LOI will receive e-mail from the project review coordinator. The agent can log into DCH CON e-Serve and click on the link ‘DCH LOI Response'. The CON No. should appear on the screen, then click on the link ‘View'. Applicable links will appear on the left hand side navigation bar - Application Components, Required Documents, Standards, Required Forms, etc. Click on each link to see the contents. Click the right mouse button on the screen to print the document.
Online Submission:
What can I submit online?
Letters of Intent, Nonsubstantive Applications, Amendments, and Emergency CONs can be submitted online. The online Substantive application module will be available Spring 2008. Projects subject to comparative review must be submitted by paper until further notice.
Submit by Another Person:
My LOI/application was submitted by another consultant. Can I access the CON response?
No, only the author (single sign-on user) can access submitted documents. If there is an author change, please contact the Department (
bhattacharyat@michigan.gov
) with the user ID so the Department can authorize a new agent.
Spell Check:
Can I do a spell check on the details that I have typed in the online LOI?
CON e-Serve does not have a spell check feature. However, you can install Internet spell check software on your computer, such as ieSpell for Internet Explorer from
www.iespell.com
. These types of software are usually free for personal use only.
Revise LOI:
I have already submitted an LOI/application to the Department. How can I revise information already submitted if needed?
Once an LOI/application is submitted to the Department, it cannot be modified. To modify, contact the Department (
bhattacharyat@michigan.gov
) with the Application ID or CON No. and the nature of the revision. The Department will unlock the documents as needed and send an e-mail. Once modified, resubmit the unlocked documents.
Letter of Intent:
Must a Letter of Intent be filed if it is believed that the proposed project does not require CON review/approval?
No. However, as a safeguard, the Department recommends that a Letter of Intent be filed as a means of avoiding any CON-related compliance issues in the future. In accordance with CON Administrative Rule 325.9201, the Department will issue a determination whether the proposed project requires a Certificate of Need.
Mergers:
If ABC, Inc., the current licensee/CON holder for a health facility or covered clinical services, merges with XYZ, Inc., and XYZ, Inc., is the surviving legal entity, is this a change in ownership?
Yes. Only ABC, Inc., has Certificate of Need approval. Section 20164 of the Public Health Code prevents a CON and/or license from being transferred, and Certificate of Need is specific to "person" and location. Therefore, XYZ, Inc., must apply for Certificate of Need review and approval prior to the execution of the merger. If approval is received and the merger is executed, then it must apply for the change in licensure, as applicable, from ABC, Inc., to XYZ, Inc.
