When will a preprinted Employer's Quarterly Wage/Tax Report (1028) no longer be mailed?
 
Answer:
There are several conditions that would cause the UIA to stop mailing the preprinted UIA 1028 to an employer.

The UIA 1028 is not mailed to employers who report this information.

It is not mailed to employers with more than 25 employees listed on the report.

If no UIA 1028 is received from an employer for two consecutive quarters or if there are no wages for two consecutive quarters or a combination of the above, then no preprinted report is mailed to an employer.

The MES Act requires an employer to file the Employer's Quarterly Wage/Tax Report by an electronic method. The schedule for implementing this requirement is as follows:

             # of Employees              Implementation Date

             25 or more                     First quarter of 2013

             6 to 24                          First quarter of 2014

             1 to 5                            First quarter of 2015

Once your implemented dates occurs there will be no pre-printed forms.
