  Wage Detail Reports (UIA 1017)
I have more than one tape. Can I put all employer information on one transmittal?
 
Answer:
A separate Form UIA 1050, Wage Data Transmittal for Magnetic Media, must be submitted in triplicate for each tape submitted in order to properly identify and process the tape(s).
