In the past, the Quarterly Wage Detail Report (UIA 1017) was preprinted and mailed to me. Why am I no longer getting the report mail to me?
 
Answer:

There are several conditions that would cause the UIA to stop mailing the preprinted UIA 1017 to an employer.

The UIA 1017 is not mailed to employers who report this information by tape or electronic transmission.

It is not mailed to employers with more than 50 employees listed on the report.

Finally, if no UIA 1017 is received from an employer for two consecutive quarters or if there are no wages for two consecutive quarters or a combination of the above, then no preprinted report is mailed to an employer.
