Software developers who intend to reproduce Michigan tax forms must complete TY09 Form 4430 Michigan Substitute Forms Letter of Intent.

Advance copies of draft tax forms and e-file specifications are published on this secure Web site. For access to this site, read Policy ET-03066 and follow the instructions for requesting a user name and password. E-mail questions to TreasForms@michigan.gov.
