Updated TR-9 Scrap Vehicle Inventory Form
The current the TR-9 (Scrap Vehicle Inventory) form security features include a red control number in the upper-right hand corner, which is used by the Department as a unique identifier. The shading includes a pantograph that becomes visible when copied. If copied, the word "VOID" will be visible in the shaded area. Copies of the TR-9 form will not be accepted.
Who May Request TR-9 Forms
TR-9 forms are available to dealers who have a Class C, H or R associated with their dealer license. All other dealers simply reassign vehicle titles directly to the scrap metal processor.
How to obtain TR-9 Forms
Orders must be in writing and include your dealer number. Orders may be submitted either by mail or fax to:

All orders will be shipped to the dealership address on file with the Licensing Unit. The TR-9 form is distributed by the pad and there are 35 forms in a pad. Dealers may order a maximum of 10 pads per calendar year. Requests for quantities over the 10-pad maximum will be handled on an individual basis. These requests must be submitted to the Dealer and Repair Resource Section of BLRD for review and approval.
