How do I get a duplicate state identification card?

If you are in Michigan, you need to take acceptable proof of identification to a local Secretary of State branch office. The fee for a duplicate identification card is $10.
If out-of-state, you need to provide the complete name (first, middle and last), date of birth, state identification number and out-of-state address. If you will be back in the state within 90 days, a temporary extension license will be mailed. If you will not be back in Michigan during that time, a temporary extension license will be mailed along with a duplicate driver license application. Applications are mailed to:
