The Department of State has replaced the TR-9 (Scrap Vehicle Inventory) form with an updated version that includes new security features.  This new form is now available and should be ordered immediately, as only the revised form will be accepted by the Department after December 14, 2007.  All previous versions of the TR-9 should be destroyed.  Licensees that continue to use obsolete forms will be referred to the Business Licensing and Regulation Division for appropriate action, up to and including suspension of the dealer's license.

Security features include a red control number in the upper-right hand corner, which will be used by the Department as a unique identifier.  The body of the form is now shaded as well.  The shading includes a pantograph that becomes visible when copied.  If copied, the word "VOID" will be visible in the shaded area.  Copies of the new TR-9 form will not be accepted.

How to obtain the new TR-9

To ensure compliance with the law, please submit your order for the new TR-9 form immediately.  Orders must be in writing and include your dealer number.  Orders may be submitted either by mail or fax to:

All orders will be shipped to the licensee address on file with the Licensing Unit.  The TR-9 form is distributed by the pad and there are 35 forms in a pad.  Dealers may order a maximum of 10 pads per year.  Requests for quantities over the 10-pad maximum will be handled on an individual basis.  These requests must be submitted to the Dealer and Repair Resource Section of BLRD for review and approval.
