How to Place a Driver's License Alert on Your Record

The purpose of the "alert" on a driver license is to limit the chance (not guarantee) that someone will get tickets in your name by using your name in traffic stops.  The alert will notify law enforcement officers that someone may be using your name and that sufficient identification should be checked whenever your name and date of birth are used during a traffic stop.  The "alert" is NOT available on State Identification cards and will only be placed on VALID Michigan driver licenses.  The address used on the "alert" form MUST MATCH the address on your driver license.

Instructions for completing the Driver's License Alert form:

  1. Click on the link below that reads DRIVER'S LICENSE ALERT and complete the following steps.
  2. Complete the area for the reason you would like the alert on your driver's license. Please be specific.
  3. You must sign and print your name.
  4. You must include your driver's license number, date of birth, address and daytime phone number (including the area code.)
  5. This form must be submitted by mail or fax to the address listed on the form.
  6. This form MUST be legible.  If illegible, request for alert will be discarded. 

Once the "alert" is placed on your driver's license:

  1. A letter will be sent to you. It is important that you keep the letter with you to show as a piece of identification if you are involved in a traffic stop.
  2. The "alert" will stay on your record for seven years. If you are still having problems with someone using your name in traffic stops after the seven years, you may request another "alert."

CAREFULLY READ AND COMPLETE THE DRIVER'S LICENSE ALERT FORM ONLY IF YOU WISH TO HAVE AN ALERT PLACED ON YOUR DRIVING RECORDS

Removing Traffic Tickets from Driving Records

If your name is being used by someone else for traffic stops and tickets have been added to your Michigan driving record, please contact the court(s) involved and request that the ticket be removed from your record. The Department of State cannot remove tickets, only a court can do this. 

Identity Theft

If you believe you may be a victim of identity theft or your name is fraudulently being used for credit purposes, contact the credit agencies involved to report the problem. There are government and private agencies that may mark their systems to protect you against further fraudulent activity.  DO NOT COMPLETE THE ALERT FORM FOR CREDIT OR CHECK CASHING PROBLEMS.  This form will not assist in preventing or stopping credit card fraud.
