To apply now, follow these steps:

• Print the Child Development and Care Application (DHS-4583).
• Complete the form.
• Sign it.

If you plan to use an in-home Day Care Aide or a Relative Care Provider who is not currently enrolled by the Department of Human Services: 

• Print the Day Care Aide Provider Application (DHS-220-A) or the Relative Care Provider Application (DHS-220-R).
• Make sure that your provider reads and meets the requirements to be a DHS enrolled day care aide or relative care provider.
• Ask your provider to complete page 1 and sign and date page 2. 

Have your provider mail or take pages 1 and 2 of the completed form to your local DHS office as soon as possible. Your provider should keep page 3.
