Apply for Child Development and Care Assistance

To apply now, follow these steps:

If you plan to use an in-home Day Care Aide or a Relative Care Provider who is not currently enrolled by the Department of Human Services: 

Have your provider mail or take pages 1 and 2 of the completed form to your local DHS office as soon as possible. Your provider should keep page 3.

Contact your local Department of Human Services (DHS) office if you need a printed Child Development and Care application. You can find the telephone number and address of your local DHS office in DHS County Offices information page.
