1. The first step is to complete the appropriate Exemption Request form. Submit the form, along with supporting documentary evidence, to the MGCB East Lansing office. Requests for exemption submitted without supporting documentation will be returned. The Supplier License Exemption Request forms may be obtained by downloading from this website or obtained by calling the MGCB East Lansing office at 517.241.0040.

2. Based on the information provided, the MGCB Executive Director will make a decision to grant or deny the request for exemption.

3. A copy of the MGCB Executive Director's decision will be forwarded to the applicant and the Board.
