An MGCB staff member or representative of the Board will verify the application.

You will be asked to:

• Sign an affidavit in which you affirm that you wish to be placed on the Disassociated Persons List.

• Authorize the MGCB to release the contents of your application to all Detroit casinos.

• Sign a form releasing the State of Michigan, the MGCB, and the Detroit casinos from any injury suffered as a consequence of participating in the Disassociated Persons program.
