1. Download as a Microsoft Word document. Complete the form on your computer and print out on your printer. After completion submit the paper form to MGCB. To use this method, you will need Microsoft Word wordprocessing software.

2. Print the PDF form on your printer. Complete and submit paper form to MGCB. To use this method, you will need Adobe Acrobat® Reader software. This software is FREE. 

Using Adobe Acrobat® to Read Documents

Many forms and other documents on Michigan Gaming Control Board site are stored as PDF (Portable Document Format) files. These files are highly compressed (to save on download times) while keeping all of the same formatting as the original document. The PDF is also a truly "cross platform" format, meaning these documents will look and work the same whether you are using a Macintosh or a Windows computer.

 

The IRS and most other states have also adopted the PDF format as a publishing standard for online documents and forms.  To view and print PDFs, you must have Adobe's Acrobat Reader (version 5.0 or better) installed.  This program is available for free download by going directly to www.adobe.com

 

Common Questions:

Some users have been experiencing problems viewing the PDF format.  If you get only a blank screen for your document, you will have to press your back button to return to the link page.  From there, right-click on the form you would like to use and save this to your computer.  The form will open correctly in Acrobat reader from there.

 

If some documents have areas that are blacked out, first try upgrading to Acrobat Reader version 5.0. If you are still unable to view or print these files directly, upgrade your printer driver on your machine and when you go to print select "Print as image".

 

If you receive a colorspace error (CS6 or CS8), it is recommended to update the printer drivers on your machine (even if you are not trying to print it will give this error). Most printer driver updates can be downloaded off the manufacturer's web site.
