Principal investigators under contract during fiscal year 2009 must complete this form for all projects that were active during the fourth quarter. First Quarter Reports are due for Statewide Planning and Research, Part II, Projects. Quarterly reports (form 5305) will cover the period October 1 through December 31, 2009 for new projects, and will add information to the previous quarterly information submitted for continuation projects. Principal investigators currently under contract must complete this form for all projects that are currently active. The quarterly reports are cumulative throughout the duration of the project.
