HIPAA Introduction

The Health Insurance Portability and Accountability Act (HIPAA) and related rules require group health plans to protect the privacy of health information. The Employee Benefits Division of the Civil Service Commission currently administers the following self-insured group health plans for state employees and retirees on behalf of the State of Michigan: Flexible Spending Accounts Plan; Preventive Dental Plan; State Catastrophic Health Plan; State Dental Plan; State Health Plan PPO; and State Vision Plan.

The "Forms" section is to be used by employees that may have a need to exercise their HIPAA rights. These rights are outlined in the Privacy Notice available under this section. Also included in this section is a Disclosure Authorization allowing the release and disclosure of personal health information and a Privacy Complaint form.
