Verification of Vital Record Facts
Key facts on any Michigan vital record can be verified. Anyone is eligible to request a verification of a Michigan vital record, as long as the information to be verified can be supplied by the applicant. State law (MCL 333.2881(2)) allows for verification of the person(s) named on the record, date of the event, place of the event and filing date. A verification does not include a copy of the vital record being verified.
Processing of a verification will include the following:
If a record is found that matches the criteria supplied, a rubber stamp will be applied to the application in the appropriate box that reads "A record has been identified which matches the supplied facts as to name, date, place and date of filing. Michigan Department of Community Health."
If a record is not found that matches the criteria supplied, a rubber stamp will be applied to the application in the appropriate box that reads "No record could be identified which matched the facts as supplied. Michigan Department of Community Health."
A request for a verification of a Michigan vital record requires a completed and signed application and payment of the required fee. The results of a verification can be faxed or mailed to the applicant.
