The department has designated all approved education program sponsors the authority to verify Medical First Responder and Emergency Medical Technician practical competencies for Michigan licensure. The attached Assurance Statement must be signed and returned as soon as possible to ensure a smooth transition for currently approved courses and those that have been completed.
Courses completing prior to 9/1/07: For those EMT students who have not completed the practical exam or who have failed the practical exam within one year of the approved course completion date must have their practical skill competencies re-verified by the program on the following form: EMT Practical Skills Re-Verification (Excel Fill-in Form)
Courses completing after 9/1/07: By signing the Practical Examination Assurance Statement and adding the student name to the Course Completion Roster, you are verifying their practical competency for Michigan licensure.
