Information for Current SLRP Providers

This section contains information and resources for current SLRP Providers.

  1.     Change of Name and/or Address Instructions

You must email MillerK3@michigan.gov to change your name and/or address with the Michigan SLRP Office.

 

In addition, you must also communicate directly with the Michigan Department of Management and Budget to change your name and/or address in their MAIN System.  Click on SLRP PROVIDER CHANGE OF ADDRESS INSTRUCTIONS, below, for a description of the steps you must take to change your records at the Michigan Department of Management and Budget.  Failure to change your address records can result in a significant delay in semiannual payments.

2.  Re-Contracting with MichiganSLRP

 

SLRP providers in good standing who have completed at least one year of their current service obligations are eligible to apply to re-contract with SLRP for a third year.

 

To apply, you must mail your Complete, Single-Submission SLRP Re-Contracting Application Package to the SLRP Office during the April 2007 Application Period with an April 2007 postmark.  All documentation must be 100% complete and legible.

 

Complete, Single-Submission SLRP Re-Contracting Application Package Requirement:

All required documents must arrive together at the SLRP Office in a single package and each document must be 100% complete and legible.  If any portion of your application is incomplete or illegible, it will not be reviewed.  Furthermore, due to the great number of applications received by the SLRP Office, we will not be able to contact you regarding incomplete or illegible portions of your application.

 

Your application package must include the following application forms.  You will find all of these forms, except number 1- Application to Re-Contract with the Michigan SLRP - Part  A, under SLRP Application Forms on this website.  You will find the  Application to Re-Contract with the Michigan SLRP - Part  A, immediately below.

 

1.      Application to Re-Contract with the MichiganSLRP - Part  A

This is a new form used only by current SLRP providers applying to re-contract with SLRP.  It replaces Part A of the SLRP Provider Application used by providers applying for SLRP for the first time.  You will notice it requires information about your current SLRP Contract and requests age-related information required for federal reporting.

2.      Part B of the SLRP Provider Application and Loan Payment History

You must include an updated Part B, along with loan payment history documentation provided by your lenders, which must clearly show that your qualifying educational debt has been reduced by the amount of all SLRP payments you have received to date.  Part B is posted under SLRP Application Forms on this website.

 

Part B of the SLRP Provider Application has not changed.  As a result, if you have recently updated your Part B which  accounts for all of your SLRP payments to date, you will not have to complete another one.  You must, however, still provide payment history documentation provided by your lenders, which must clearly show that your qualifying educational debt has been reduced by the amount of all SLRP payments you have received to date. 

 

Please note: You will also be required to submit another Part B and payment loan history from your lenders that clearly shows that your qualifying educational loans were reduced by at least the  amount of all of your SLRP payments under your current contract before you will receive your first check under any new contract.  It is your responsibility to remember to submit this additional Part B and loan payment history to the SLRP Office.  The SLRP Office will not remind you to do this.  Failing to so will result in a significant payment delay.

 

Special Instructions for Part B:

Please note that Part B of the Provider Application takes considerable time to complete.  First, you must complete Sections l and ll and mail or fax it to the organizations that hold your loans. You must ask them to complete Sections III, IV and V and then mail or fax it back to YOU.  You must then receive the completed Parts B back in time to include them with your Complete, Single-Submission SLRP Re-Contracting Application Package, which you must mail to the SLRP Office with the required April 2007 postmark.  This process can take several weeks, so it makes sense to get an early start on Part B.

 

Do not instruct your lenders to send their completed Parts B to the SLRP Office.

It is your responsibility to include completed Parts B in your Complete, Single-Submission SLRP Re-Contracting Application Package which you must mail to the SLRP Office.

 

Please note that you may save a great deal of time by faxing Part B to your lenders and having your lenders fax it back to you.  Previously faxed Parts B are acceptable, as long a they are legible and arrive at the SLRP office by mail as part of your Complete, Single-Submission SLRP Re-Contracting Application Package.  Application forms faxed directly to the SLRP Office will be discarded without notification to the applicant.

 

 

3.      SLRP Practice Site Application and Declaration of Intent

You must also include an updated SLRP Practice Site Application and Declaration of Intent, completed and signed by your employer, as part of your application package.  This is a new form that replaces and combines the previous SLRP Site Application and Letter of Intention.  This new form is posted on this website under SLRP Application Forms.  The SLRP Office will review this form for your employer's declaration of intent to employ you during the term of a new SLRP Contract, should your application to  re-contract prove successful, and to determine if your Practice Site is currently located within a Health Professional Shortage Area and meets all program requirements.

 

 

Mailing  your Complete, Single-Submission SLRP Re-Contracting Application Package

Because we need original signatures, you must mail your application package to the SLRP Office.  Application packages faxed to the SLRP Office will be discarded without notification to the applicant.  Applications received with postmarks before of after what is required will be returned by mail.

 

Before submitting your application, make certain you have included all required applicationmaterials and that each document is 100% complete and legible.  Applications not meeting the Complete, Single Submission SLRP Re-Contracting Application Package Requirement will not be reviewed.

 

Your Complete, Single-Submission SLRP Re-Contracting Application Packagemust be mailed with an April 2007 postmark  to:
