SLRP Application Periods

 

Up to two SLRP Application Periods will be available each year, during which eligible health care providers may submit their Complete, Single -submission SLRP Application Packages- one during the month of April, and a possible second during the month of November.

 

April Application Period 

The Michigan Department of Community Health encourages all eligible health care providers to apply during the April Application Period.  The April Application Period may be the only application period offered during any year, so you should plan your application accordingly.

 

If you decide to apply for SLRP during the April Application Period, your Complete, Single-submission SLRP Application Package must be mailed to the SLRP Office with an April postmark for that application year.  Applications received with postmarks before of after what is required will be returned by mail. 

 

During the April Application Period, providers will be able to apply for one of the three types of SLRP Contracts: Competitive Contracts, Local Match Contracts; or CMH/DOC State Match Contracts.  All of these types of contracts are explained above under SLRP CONTRACTS.

 

The SLRP Office will review all applications that meet the Complete, Single-submission SLRP Application Requirement, described below, and notify applicants selected for SLRP Contracts by the following July 31st .   Unsuccessful candidates will be notified as soon as the SLRP Office confirms that each candidate selected agrees to participate in the program.  Candidates not initially selected will remain on a waiting list and will be offered contracts as candidates already selected decline participation or are determined to be ineligible for the program.  Selection of these additional participants will be based on the selection criteria and process described above.

 

Successful applicants from the April Application Period will receive contracts starting on the following October 1st.  Copies of the SLRP contract will be sent to each successful applicant for review, signature, and return to the MDCH.  A final, fully-executed contract will be sent to the new SLRP provider, and to the employer if a Local Match or CMH/DOC State Match Contract, after MDCH signatures are obtained.

 

November Application Period

A November Application Period will be offered if an insufficient number of candidates apply during the April Application Period to commit all of that year's Federal Grant Award and/or State funds.  The nature of the November Application Period, if any, will depend on the type and amount of SLRP funds still available.  If both Competitive Contract funds and Match Contract funds remain available, the November Contract Period will look much like the April Contract Period, only smaller.  If only Competitive Contract funds or Match Contract funds remain available, the November Application Period will only include applications for the type of contract with remaining funds.  The selection criteria, process and rules described above will apply.

 

If a November Application Period is offer and you decide to apply, your Complete, Single-submission SLRP Application Package must be mailed to the SLRP Office with a November postmark for that application year.  Applications received with postmarks before of after what is required will be returned by mail.
