National Physical Therapy Examination (NPTE) & Michigan Jurisprudence Exam
To become licensed in Michigan, an applicant must request a licensure application packet by email at bhpinfo@michigan.gov, by contacting the Department of Community Health, Bureau of Health Professions at (517) 335-0918 or download the application packet (see physical therapy homepage under forms and applications).
The licensure application packet includes the application for licensure, the NPTE Handbook, and the Michigan Jurisprudence Examination.
To be made eligible to take the National Physical Therapy Examination (NPTE), the licensure application, the Michigan Jurisprudence Examination, licensure fee and required documentation must be completed and submitted to:

The National Physical Therapy Examination (NPTE) is a computer-based examination given at Prometric Testing Centers. Applicants for the NPTE must apply online. Online application and payment for the NPTE are available at www.fsbpt.net/pt. See the NPTE Candidate Handbook for complete instructions or check the website at www.fsbpt.org.
Once an applicant has been made eligible for the NPTE, the Bureau of Health Professions will notify NPTE. NPTE will then send the applicant an Authorization to Test and information on how to contact one of the Prometric Testing Centers to schedule an appointment to test.
Results are reported approximately six weeks following the examination date.
