Once an agreement is in place, a referral is made by completing the Ticket to Work Referral Form. This form must be accompanied by the following documentation, all of which comprises the Referral Packet:
- Copy of a Completed and Signed MRS Application/RA-2910
- Signed Release of Information Forms Which Will Allow Us Free Exchange of Information
- Verification of Ticket Eligibility and Assignment (Copy of Letter from Maximus Assigning Ticket)
- Copy of the Completed and Signed Individualized Work Plan (IWP)
