How Do I Apply?

Application

The first steps in applying for MRS Services are:

  • Attending an orientation/intake/interview for an overview of MRS.
  • Please ask questions
  • You'll be given an opportunity to complete an MRS application.  This means you are applying for assistance from MRS with a goal of employment. 

After orientation and filling out the application MRS will need to determine your eligibility.  This process is conducted by appointment with an MRS counselor.

IMPORTANT INFORMATION

Please bring the following information to your appointment:

  • Original Social Security
         Work Visa - If applicable
  • Driver's License or State Identification
  • A list of your treatment providers including their names, address and phone numbers.
  • A list of names and dosages of medication(s) you are currently taking.
  • Verification of SSI, SSDI, SDA, DHS and/or unemployment or other benefits you are receiving.
  • Employment data including employer names, addresses, dates of employment, wages, reason for leaving and job duties for your last three jobs. 
  • Resume if you have one.
