The Michigan Primary Care Association (MPCA) Community HealthCorps program in 2009-2010 will engage 17 full-time and 4 half-time AmeriCorps members to address patient assistance, school-based health, and health outreach and enrollment. In addition, the MPCA Community HealthCorps program is engaging 15 full-time American Recovery and Reinvestment Act members to do health outreach and education.
Members are placed in community health centers and other host sites across the state. The MPCA Community HealthCorps program uses community outreach to inform the public about health plans available to the uninsured and preventative health care measures. AmeriCorps members also assist patients so they become more active in the management of their illnesses and reduce health disparities in Michigan. Community forums are used to inform the public about adult, adolescent, and children's health issues.
2009-2010 Corps members will work with 24,000 patients each year, with 60% of identified chronically-ill patients establishing at least one self-management goal, will help 8,313 uninsured and underinsured Michigan residents gain information through community forums and ongoing literature distribution, and will enage 375 volunteers in 1,500 hours of community service.
