I'm a Lottery prize winner who has moved. How do I request a duplicate W2-G to be mailed to my new address?
 
Answer:

This type of request must be submitted in writing, providing the new address, your name, printed and signed, social security number, and daytime phone number.  This information can be sent to Michigan Lottery, Attention Administration, PO Box 30023, Lansing, MI 48909, or faxed to 517-241-7506.  Requests not containing all of this information cannot be processed.
