MAPS Information for Veterinarians
All controlled substances dispensed by a practitioner, including veterinarians, are required to be reported electronically through MAPS Online on the 1st and 15th of every month. MAPS Reporting Requirements of Prescription Data
are expected to change from twice monthly to daily reporting in mid to late 2013 in an effort to provide more accurate and up-to-date prescription information to health professionals. Registration to MAPS Online
is required to submit prescription data and to request controlled substance history reports.
PRESCRIPTION DATA FOR ANIMALS
When entering prescription data into MAPS for controlled substances dispensed for animals, please keep in mind the following tips:
- Customer ID: Use the pet owner's identification (driver's license or Michigan I.D. card) for all controlled substances dispensed for animals.
Birth Date: Use the pet owner's date of birth.
Patient First Name: Use the pet owner's first name.
Patient Last Name: Use the pet owner's last name.
NDC Code: Entry requires 11 digits.
Rx Number: The system will not accept letters or dashes (-,/); enter only a maximum of 7 digits.
