PHYSICIANS AND PHARMACISTSRequests for MAPS Reports are accessible 24/7 through MAPS Online and reports are usually available within minutes of a request. Physicians and pharmacists who do not have online access may use the form below to request a MAPS report via fax. Please note that faxed requests for MAPS reports take, on average, 48 hours to process.
A wavier may be requested by dispensers who do not have the appropriate software to record prescription data. Upon approval of the waiver application, a MAPS Claim Form is required to be completed for all controlled substances dispensed and then mailed to MAPS for submission into the database.
LAW ENFORCEMENT
Access to MAPS reports is available to law enforcement agencies through MAPS Online. The following form must be completed and mailed to MAPS to begin the online registration process. Please note that faxed or emailed forms cannot be accepted.
Law enforcement agencies may request a MAPS report by fax using the below form if a MAPS report is needed during the online registration process.
