Upon
Registration to MAPS Online and for requests for MAPS reports, practitioners are required to acknowledge and certify that the information they are requesting shall be used only for the purpose of providing medical or pharmaceutical treatment to a
bona fide current patient and that they will not provide this information to any other person or entity except for other practitioners who are treating the same patient. HIPAA and all confidentiality and disclosure provisions of Michigan Law cover the information obtained in the MAPS database. These requirements include, but are not limited to, logging into MAPS using your individual user id and password, and not sharing your user id and password with others.
