How to File a Complaint Against Health Facilities
The Facilities Complaint and Investigation Section receives and processes consumer complaints against nursing homes, home health agencies, hospices, hospitals, freestanding surgical units, ambulatory surgical units, and end-stage renal dialysis centers. THREE METHODS FOR FILING A COMPLAINT
1. Completing the BHCS Health Facility Online Complaint Form. THIS FORM IS ONLY FOR INDIVIDUALS THAT WANT TO FILE A COMPLAINT.
2. Calling the toll-free Complaint Hotline: 800-882-6006
3. Mailing or faxing a completed Resident/Patient Care Complaint Form (BHCS-LTC-361), that can be obtained by clicking on the form name and printing, or by submitting a letter that includes:
-- The name and the address of facility.
-- The nature of the complaint.
-- The date of the incident.
-- The complainant's name, address, and daytime telephone number.
