Maintaining Your Radiation Machine RegistrationIf you currently have a radiation machine registration certificate, here is how to keep your certificate up-to-date. Adding, deleting, or changing machine information: A facility that needs to add or delete an x-ray machine from an existing certificate may contact the Radiation Safety Section at 517-241-1989, fax a copy of the current certificate with machine changes to 517-241-1981, or e-mail the machine changes to rssinfo@michigan.gov. When adding a new machine, be sure to include all the information that is required on the certificate. Changing the Radiation Protection Supervisor: The radiation protection supervisor change needs to be made in writing, either via fax at 517-241-1981, e-mail to rssinfo@michigan.gov, or by regular mail. Relocating the machine to another location at a different facility address: If the machine is being relocated to another facility that has an existing certificate, call the Radiation Safety Section at 517-241-1989 to register at the new location. If the machine is being relocated to another facility that does not have an existing certificate, the completion of a Radiation Machine Registration Application and submission of a registration fee is required for that new location. Call the Radiation Safety Section at 517-241-1989 to request the application or you may register online. Relocating the machine to another location within the same facility (facility address does not change except maybe a suite # or room #) Call the Radiation Safety Section at 517-241-1989 to change the location within the facility on the certificate, fax a copy of the current certificate with room/suite change to 517-241-1981, or e-mail the room/suite change to rssinfo@michigan.gov. Depending on the type of x-ray machine, an Application for a Radiation Shielding Plan Review should also be submitted, if not already submitted. Refer to our website for more information on which machines require a radiation shielding plan review. Change of ownership of a radiation machine: The completion of a Radiation Machine Registration Application and submission of a registration fee is required for the change of ownership. Call the Radiation Safety Section at 517-241-1989 to request the application or you may register online. Certificate is expiring within 1 ½ months and our office hasn't received the renewal application: Please call the Radiation Safety Section at 517-241-1989 to request that another renewal application be sent to you. Our office does not have a copy of our current Radiation Machine Registration Certificate: Call the Radiation Safety Section at 517-241-1989 to find out if the certificate is current. If the certificate has expired, a renewal application will be sent. If the certificate is current, (and all information on record is correct), another certificate can be issued.
