Owner / Breeder Registration

In order for the Michigan Department of Agriculture (MDA) to process payment for Owners/Breeders' Awards, a payee must be registered on the new state-wide vendor/payee file in a format which is in accordance with IRS regulations.

To complete the registration process, fill out ALL THREE of the following forms:

  1. Breeder / Owner Registration Form
  2. Payee Registration Form:  Complete boxes one through six as requested.
    • Block 1-2:  In accordance with IRS regulations, the name and number must match existing IRS records.
    • Block 4:  If payment is to be made to individuals in addition to the name listed in Box 2, those additional names should be listed in the "ATTN" sections of box 4.

  3. IRS Form W-9 Form:  Complete in accordance with the instructions on the form.  The name on the W-9 must match the name on the payee registration.  If the payments are being received under an individual’s social security number, please use only that individual’s name on the W-9, and mark the square individual/sole proprietor even if payment is made in more than one name.
  4. Mail the three forms to:
