The online Enhanced and Excellent Levels have not been updated to reflect the 2007 revisions. Please plan based on the Enhanced and Excellent Measures posted below. The Library of Michigan will announce when the online levels have been updated to match.
The history and background of QSAC is in Introduction to Quality Measures.
For a current list of QSAC certified libraries, go to the Interactive Library Directory and search by QSAC Level.
Instructions:
1. Log in using your annual report FSCS ID
sent to you by the Library of Michigan. Please contact the QSAC coordinator at
HAL-QSAC@michigan.gov for your password.
2. Start with the Essential Level, filling out the completion date for each item
in each of the six areas of service. You may use either the date when the measure was actually completed or the current date. Comments are optional.
3.
Upon completion, submit the form electronically, print out and sign the
Certification Form and mail to the address listed on the form.
4. The Library of Michigan will send you a certificate verifying that
your library has achieved Essential Level status. A sample press release for
you to adapt for your local media can be found at the following link:
QSAC Sample Press Release
(MS Word Format).
5.
After you receive your Essential Level Certification from the Library of
Michigan, you may begin the same procedure for the Enhanced Level. When you
receive Enhanced Level certification, you may continue on to the Excellent
Level.
