My children need health insurance. I have a job, but my employer doesn't provide coverage and I can't afford to buy private insurance. Where can I get information regarding our eligibility for coverage?

Your children may be eligible for the MiChild health insurance program. MIChild is for uninsured children of Michigan's working families. To qualify, children must:
- Be citizens of the U.S. (some legal immigrants qualify)
- Live in Michigan, even for a short time
- Be under 19 years old
- Have no health insurance
- Live in a family with monthly income of about (or less):
$1,800 for a family of two
$2,300 for a family of three
$2,800 for a family of four
$3,200 for a family of five
Coverage will cost $5 per family per month, for a total of $60 per year.
Click on this link for an application form that you can print and fill out. Applications are also available from most community human services agencies or by calling: 1-888-988-6300
Families need only fill out a short MIChild application. Submit the application with proof of income (for example, copies of the previous year's W-2, a recent check stub or recent bank statements).
